Does anyone have a sample film inspection report they might be willing to share?

All of ours have mysteriously disapeared, and I can't find an original anywhere.

Always a good idea to keep an "inspection report" for each print, with comments before, during, and after the run. Helps track problems (e.g., if a scratch was there when the film came in, you won't chase your tail looking for the cause when you spot it during the run). Also, if you have documented damage on the film as received, it helps if you are later unjustly blamed for it. (Always report severe damage to the distributor immediately, and request replacement reels if your show will be compromised).

Likewise, an equipment maintenance log is invaluable.

I require one to be filled out on reviseing of each print on arrival before the first screening. That is the only way to cover your butt
